Generating OSHA and other Industry-Standard Reports
Objective
In the United States, OSHA (Occupational Safety and Health Administration) governs workplace safety and requires that certain forms be completed and filed after workplace accidents. Failure to file these reports can result in severe fines and penalties. Outside of the U.S, other countries require that similar types of accident reports be filed.
Solution
With the Archibus Health & Safety system for reporting and tracking workplace accidents ( incidents ), you generate all the data necessary for completing several OSHA reports. The Health & Safety system offers a pre-formatted report that emulates one OSHA form. For other OSHA forms, you can use Archibus tools for generating your own formatted reports from standard Archibus Health & Safety reports.
In addition to OSHA reports, you may need to file reports for non-US regulatory agencies, workers' compensation, CAPA, and so forth. You can use the standard Health & Safety reports to meet these needs, and if necessary, modify these reports if you require specific formatting.
Procedures
Generate OSHA Accident Report, Form 301
This report must be filed when workplace accidents occur. You can directly generate this report from several Health & Safety incident-related tasks and reports by simply by selecting the OSHA Accident Report button from on the task. The system generates a PDF that emulates the OSHA Accident Report (Form 301) and completes the various fields with data from the Archibus system.

Generate OSHA Log of Work-Related Injuries and Illnesses, Form 300
OSHA Form 300 lists all work-related injuries for a calendar year and categorizes the severity of each case. Companies in the US must yearly complete this report and note any workplace injuries that meet OSHA criteria.
The Archibus Incidents Log report presents all the data that you need for this OSHA report.
- Run the Management Reports / Incidents Log Report.
- Use this report's OSHA Log Report action button to generate the OSHA Log Report, From 300 .
Generate OSHA Summary of Work-Related Illnesses and Injuries, Form 300A
The OSHA Summary of Work-Related Illnesses and Injuries, Form 300A, summarizes the incident data throughout the year. This report is typically posted in the workplace so that employees are apprised of the accident rate at their workplace.
The Archibus Incidents Log report provides the same information as required by Form 300A.
- Run the Management Reports / Incidents Summary Report.
- Use this report's OSHA Summary Report action button to generate the OSHA Summary Report, Form 300A .
Generate Other Incident-Related Industry Reports
Archibus reports have been designed to include all information typically required by industry and government reports. When filing safety-related reports, you will find that the Health & Safety reports typically provide all the information you need. There are a few ways you can use the Health & Safety reports to meet your external reporting needs.
- You may be able to submit a standard Archibus Health & Safety report in place of completing a government or industry form.
- You can generate a Word document from incident data and then use Word to edit and format the data to match the form you need to complete. For example, you might generate a Word document to create a Worker's Compensation report or a "Near Miss" report.
