Request Portal - Creating a Request

After you have received the URL and navigated to the portal, you will see a similar form:

The required fields will be marked with an asterisk, these fields must be completed in order to submit the request.

• Entity
  • Formerly known as the Facility in Hippo
• Asset
  • Formerly known as the Equipment, Location, or Vehicle in Hippo
• Subject
  • ​​​​​​​Brief summary of the work being requested
• Description
  • ​​​​​​​Detailed summary of the work being requested (optional)
• Email
  • ​​​​​​​Requester's email

To submit a request from the portal, complete the form and then click the Submit button.  Your new request will now be available in the Requests Management screen.
